Client Forms

• Forms may be printed and completed in advance of first visit. You will need one Intake, one *Client Policies, and one Informed Consent form per client. Please be sure to select the proper intake forms for Prenatal or Minor Child when applicable. *If using the form for Minor Child, the additional Client Policies form is not necessary.

• Please be sure to bring all appropriate forms with you.  Otherwise, please arrive to your first appointment about 10-15 minutes early to complete them on site.
