Client Forms

• Forms may be printed and completed in advance of first visit. You will need one Client Intake,  one Policies, and one Informed Consent form per client.

• HIPAA Notice may also be downloaded here for your records.

• Please be sure to bring all appropriate forms with you.  Otherwise, please arrive to your first appointment about 10-15 minutes early to complete them on site.
